Prime Minister Tan Sri Muhyiddin Yassin announced that the government will enforce the Movement Control Order (MCO) in the Federal Territories and five other states from Wednesday, 13 January 2021 until 26 January 2021 to curb the spread of Covid-19. Besides, Conditional Movement Control Order (CMCO) will be enforced in six other states for the same period of time.
For the public who wishes to travel interstate and inter-district during the period of MCO and CMCO, it is required to apply for permission to travel by submitting certain forms to the nearest police station.
Failure to produce or show the required documents to the police officer in a roadblock may cost the public a fine not exceeding RM1,000.00 or to imprisonment for a term not exceeding six months or to both.

1. Download the official forms provided above.
2. Fill in the form in 2 copies
3. Submit the 2 copies to the nearest police station for application and approval.
